29 CFR 1910.307(c)(2)(i): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location:    (a)  CWB Holdings, Inc. at 2425 55th Street Suite 100, Boulder, CO 80301: On and preceding 4/21/17, the employer did not ensure that equipment, wiring methods, and installations of equipment in hazardous (classified) locations were intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location. Employees performed hemp extraction using isopropyl alcohol.  The computer, duplex outlets and rotary evaporators located within the IPA extraction room are not approved for a Class I Division 1 location. This condition exposed employees to a flash fire hazard due to the potential for an ignition source in a room where flammable vapors are present.      Abatement Note:  Among other methods, appropriate abatement methods include:  (1) Installation of engineering controls, such as chemical fume hoods, which exhausts flammable or combustible vapors from the room.

Employees performed hemp extraction using isopropyl alcohol.  The employees were required to wear 1/2 face tight fitting air purifying respirators with organic vapor cartridges.  The employer did not have a written respiratory protection program.  This condition potentially exposed the employees to a respiratory hazard.

Employees performed hemp extraction using isopropyl alcohol.  The employees were required to wear 1/2 face tight fitting air purifying respirators with organic vapor cartridges.  One employee had significant facial hair between the facepiece and the face. This condition potentially exposed the employee to a respiratory hazard

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve:  (a)  CWB Holdings, Inc. at 2425 55th Street Suite 100, Boulder, CO 80301: On and preceding 4/21/17, the employer did not ensure respirators were stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were packed or stored to prevent deformation of the facepiece and exhalation valve.  Employees performed hemp extraction using isopropyl alcohol.  The employees were required to wear 1/2 face tight fitting air purifying respirators with organic vapor cartridges.  Employees hung their respirators in the isopropyl alcohol extraction room.  This condition potentially exposed the employees to a respiratory hazard.

The employer had not developed a written hazard communication program with regards to labeling and safety data sheets of hazardous chemicals in their work area.
