OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to burn hazards: (a) On or about 4/10/2017 - Ductile Pipe #1 Casting Area, trough operators receiving molten metal from iron pourers were exposed to molten metal that can overflow when being transferred from the Holding Ladle into the Quadrant Ladle.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) On or about 4/10/2017 - Ductile Pipe #1 Casting Machine, employees working on the ductile iron pipe were not provided the appropriate spatter guards or "spats" for protection against molten iron at approximately 2600 degrees F.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment: (a) On or about 4/10/2017 - Ductile Pipe #1 Casting Machine, the personal protective assessment for trough operators did not identify the appropriate foot protection including spatter guards or "spats" for employees exposed to molten metal at approximately 2600 degrees F when working on the ductile iron pipe.
