29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of carbon monoxide listed inTable Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 parts per million:    Inter City Maintenance Services, Inc., Chicago, IL- the employer did not ensure that employees were not exposed to a concentration(s) of carbon monoxide in excess of the OSHA Permissible Exposure Limit  of 50 parts per million (ppm) as an 8 hour Time Weighted Average Concentration.     a) Inter City Maintenance Service, Inc.- On or about April 22, 2017, an employee using a gasoline powered spray washer at "The Promenade" condominium building, 1201 W. Adams, Chicago, was exposed to an 8-hour time-weighted average (TWA) concentration of carbon monoxide of 83.9 parts per million (ppm). This is in excess of and is 1.68 times the OSHA 8-hour Time Weighted Average (TWA) Permissible Exposure Limit of 50 ppm. The occupational exposure was measured over 160 minutes. Zero exposure was assumed for a period of 320 minutes. The carbon monoxide permissible exposure limit is established to prevent headaches, nausea, unconsciousness and asphyxiation.    The carbon monoxide exposure level was derived from employee carboxyhemoglobin levels of 20%, measured on April 22, 2017.     b) Inter City Maintenance Service, Inc.- On or about April 22, 2017, an employee using a gasoline powered spray washer at "The Promenade" condominium building, 1201 W. Adams, Chicago, was exposed to an 8-hour time-weighted average (TWA) concentration of carbon monoxide of 85.7 parts per million (ppm). This is in excess of and is 1.71 times the OSHA 8-hour Time Weighted Average (TWA) Permissible Exposure Limit of 50 ppm. The occupational exposure was measured over 160 minutes. Zero exposure was assumed for a period of 320 minutes. The carbon monoxide permissible exposure limit is established to prevent headaches, nausea, unconsciousness and asphyxiation. 29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d):   Inter City Maintenance services, Inc., Chicago, IL- On or about April 22, 2017, effective engineering and administrative controls were not instituted and maintained by the employer to reduce employee exposure to carbon monoxide during use of a gasoline powered pressure washer in an enclosed area.   a) On or about April 22, 2017, an employee was exposed to carbon monoxide at an 8-hour TWA level of 83.9 ppm, approximately 1.68 times the OSHA PEL of 50 ppm.    b) On or about April 22, 2017, an employee was exposed to carbon monoxide at an 8-hour TWA level of 85.7 ppm, approximately 1.71 times the OSHA PEL of 50 ppm.   c) On or about April 22, 2017, an employee was exposed to  carbon monoxide at an 8-hour TWA level of 87.1 ppm, approximately 1.74 times the OSHA PEL of 50 ppm.   See instance description in item 1a of this citation.   Methods of engineering and work practice controls include but are not limited to:   1) Replace the gasoline powered pressure washers with electric or pneumatic pressure washers.   2) If gas-powered pressure washers are necessary to use, evaluate the work space and ensure adequate ventilation, especially in enclosed work spaces.   3) Use suitable carbon monoxide detectors or alarms to warn workers of buildup of carbon monoxide when using gas powered equipment such as pressure washers.    Step 1  A written detailed plan of abatement shall be submitted to the Area Director outlining a schedule for implication of engineering and or administrative measures to control employee exposures to carbon monoxide from the gas-powered equipment. The plan shall include, at a minimum, target dates for the following actions which must be consistent with the dates required by the citation.   1) Evaluation of engineering/administration control options;   2) Selection of optimum control methods and completion of design;   3) Procurement, installation and operation of selected control measures;   4) Testing and acceptance of control methods. All control methods shall be evaluated for each particular use by a technically qualified person.    Abatement Date: 10/18/17   Step 2  Abatement shall been completed by the implementation of feasible engineering and work practice controls and verification of their effectiveness in achieving compliance.    Abatement Date: 11/8/17   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    a) On or about April 22, 2017, Inter City Maintenance Service, Inc., Chicago, IL, did not develop, implement, and/or maintain at the workplace a written hazard communication program in accordance with 29 CFR 1910.1200(e)(1) that would at least describe or include how the following would be met:    1) Requirement for labeling and other forms of warning;  2) Safety data sheet availability;  3) Employee information and training;  4) A list of hazardous chemicals known to be present in the workplace;   5) Methods to inform employees of the hazards on non-routine tasks; and   6) Methods to provide other employer(s) access to safety data sheet; information on any precautionary measures and the labeling system used in the workplace. 29 CFR 1910.1200(h)(1): The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees had not previously been trained about was introduced into their work area and chemical-specific information was not always available through labels and material safety data sheets:  a) Inter City Maintenance  Service, Inc., Chicago, IL- On or about April 22, 2017, the employer did not ensure that employees were provided training when they were exposed to hazardous materials, such as, but not limited to carbon monoxide from gasoline powered pressure washer to clean an enclosed parking garage at the jobsite The Promenade, 1201 W. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
