29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a) At 229 Dozier Dr. Bldg.6, Troy, Alabama:  On or about April 24, 2017 and at times prior; the employer allowed his employees to work from pump jack scaffold platforms without fall protection exposing them to a fall hazard of up to 40 feet to the ground below.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary:   a) At 229 Dozier Dr. Bldg.6, Troy, Alabama:  On or about April 24, 2017 and at times prior; the employer allowed his employees to work from pump jack scaffolds that did not have its poles secures at the bottom exposing the employee to a fall hazard of up to 40 feet to the ground below.
