29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees.        In the cracking room, an employee's fingers were caught in a stick tight breaker machine while clearing a stick jam without de-energizing and applying a lock or tag on the machine exposing him to caught-in hazards.
