29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about April 24th, 2017, the employer failed to ensure that machine specific procedures were developed and followed where employees were changing out the blade of the Powermatic Houdaille table saw(Serial #797200511), located in the saw room. Employees are changing out the saw blade by turning off the machine but not turning off the main disconnect switch or locking out any components of the table saw or at the main disconnect. The last time a saw blade was changed out in the manner was sometime in February of 2017.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about April 24th, 2017, the employer failed to ensure that employees were applying lock out tag out procedures when changing out the blade of the Powermatic Houdaille table saw(Serial #797200511), located in the saw room. Employees are changing out the saw blade by turning off the machine but not turning off the main disconnect switch or locking out any components of the table saw or at the main disconnect. The last time a saw blade was changed out in the manner was sometime in February of 2017.

29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee who wore prescription lenses while engaged in operations that involved eye hazards wore eye protection that incorporated the prescription in its design, or wore eye protection that could be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses. On or about April 24, 2017, the employer failed to ensure that employees who were required to wear protective eyewear were wearing safety rated prescription eyewear or safety eyewear that fits over top of their personal prescription eyewear. Employees were found wearing non safety prescription eye wear with ineffective slip on side shield guarding in place.
