29 CFR 1926.152(d)(4): At least one portable fire extinguisher having a rating of not less than 20B:C units was not provided on all vehicles used for transporting and/or dispensing flammable or combustible liquids:    The employer did not ensure that a portable fire extinguisher was available to employees who were carrying a 5 gallon gasoline can in the vehicle. The gasoline was being used to fill the air compressor.

29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of subsection 1926.502:    The employer did not ensure that fall protection was implemented when employees were removing and installing asphalt shingles on a residential structure, exposing them to fall hazards from 10 feet and greater.     Yoni Perez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1183739, citation number 1, item number 1 and was affirmed as a final order on 11/16/2016, with respect to a workplace located at 1094 Treeshade Dr., St. Peters, MO  63376.
