29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) SNS Absolute Construction, LLC on April 25, 2017 at the address of 4150 W. Laskey Rd. in Toledo, Ohio: Employees were exposed to falls of approximately nine feet in that an extension ladder used to access the roof of a commercial structure was used without extending the ladder's side rails at least three feet above the landing surface.

29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) SNS Absolute Construction, LLC on April 25, 2017 at the address of 4150 W. Laskey Rd. in Toledo, Ohio: The employer did not have a plan or program to identify job site hazards for employees to maintain a safe working environment.  Such a program would allow the employer to identify potential hazards such as falls during job site operations, decreasing the probability of employee injury.   SNS Absolute Construction, LLC was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1926.20(b)(1), which was contained in OSHA inspection number 1194279, citation number 1, item number 1a and was affirmed as a final order on March 29, 2017, with respect to a workplace located at 1740 E. Wooster St., Bowling Green, OH 43402.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) SNS Absolute Construction, LLC on April 25, 2017 at the address of 4150 W. Laskey Rd. in Toledo, Ohio:  The employer failed to ensure that employees were protected from falls of up to approximately nine feet while performing roofing work to a commercial structure.     SNS Absolute Construction, LLC was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.501(b)(10)  which was contained in OSHA inspection number 1194279, citation number 1, item number 1b and was affirmed as a final order on March 29, 2017, with respect to a workplace located at 1740 E. Wooster St., Bowling Green, OH 43402.
