29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition.  On 4/25/17, at the Pilgrim's Pride, Gainesville facility, in the Picking room, employees were exposed to trip hazards as drain covers were not maintained in a safe condition. Areas included:  a - Areas at Line 2 were missing covers and some drain covers were too wide to provide safe crossing and had openings up to 7 inches.   b - Areas around the Line 2 Quill Puller were not guarded with covers.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points, rotating parts, flying chips and sparks.  On 4/17/2017, at the work station located at 920 Queen City Parkway, Gainesville, Georgia, employees were exposed to amputation hazards as they were directed to clean the Line 2 Quill Puller that was not guarded.

29 CFR 1904.29(b)(3): Each recordable injury or illnesses was not entered on the OSHA 300 log and/or incident report (OSHA Form 300A or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness has occurred.  On 4/17/2017, at the Pilgrim's Pride facility located at 920 Queen City Blvd, Gainesville Georgia, an employee incurred an amputation and the injury was not recorded on the OSHA 300 log at the time of the site inspection on 4/25/2017.
