29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.   Location: front of building On or about 4/24/17,  an employee accessing a platform on a fabricated frame scaffold 24 feet above a lower level was not provided a means to access the platform.

29 CFR 1926.451(g)(1)(iii): Each employee on a crawling board (chicken ladder) was not protected by a personal fall arrest system, a guardrail system (with minimum 200 pound toprail capacity), or by a three-fourth inch (1.9 cm) diameter grabline or equivalent handhold securely fastened beside each crawling board;  Location: roof On or about 4/24/17, an employee was not protected from falling while using a chicken ladder to access a platform at the roof peak.

29 CFR 1926.454(a)(3): The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the proper use of the scaffold, and the proper handling of materials on the scaffold.     Location: site  On or about 4/24/17, the employer did not provide training for an employee accessing and working from fabricated frame, crawling board (chicken ladder) and roof bracket scaffolds.
