29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:    (a) Sixto Medrano DBA Big Six Roofing on or about April 25, 2017, at the address of 1225 Cleveland Rd. Sandusky, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

Sandusky, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing various roofing operations from elevated surfaces.

Sandusky, OH: The employer did not ensure employees were protected from falls while installing new roofing material to a residential type structure.  Employees operating in this condition were exposed up to a 12 ft. fall to the ground below.
