Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On 4/25/2017, Jose Gallardo was installing shingles on a roof of a commercial building located at 2031 Collier Corporate Parkway in St. Charles, MO. They were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. The owner and three employees were observed on the 6/12 sloped roof approximately ten feet above the ground without fall protection, exposing them to a fall to the ground below.
