(a) On or about 04/24/17, at the Town Center Apartments, located on Calkins Road, in the town of Henrietta, New York, employees were exposed to falls from stepping into holes from a broken pallet used as a platform for a "Frost Fighter" temporary heater, located outside the "club house". NO ABATEMENT CERTIFICATION REQUIRED
