29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:     The employer is failing to ensure that all employees exposed to fall hazards while working on a steep-sloped commercial roof are utilizing fall protection.  This most recently occurred on April 17, 2017 on the jobsite located at 5323 N 134th Ave, Omaha, NE 68164.  Employees were observed re-roofing a building with an 8 in 12 sloped roof at heights of approximately 9 feet above the ground.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.    The employer is failing to ensure that each employee who might be exposed to fall hazards has been effectively trained on how to use and operate personal fall arrest systems.  This most recently occurred on April 17, 2017 on the jobsite located at 5323 N 134th Ave, Omaha, NE 68164.  Employees were observed reroofing a residence on an 8 in 12 steep sloped roof at heights greater than 6 feet above the ground below without utilizing their personal fall arrest system.

29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level:     The employer is failing to ensure that all employees are protected from fall hazards when using two ladders to access and egress a commercial roof.  This most recently occurred on April 17, 2017 on the jobsite located at 5323 N 134th Ave, Omaha, NE 68164.  The employer had employees using an extension ladder to gain access to the roof of a commercial building for re-roofing activities.  The ladder being used was placed on an uneven surface and not secured to the building.  The uneven surface caused the ladder to lean to the left which could cause the ladder to shift and fall when used.

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders:     The employer is failing to train employees to recognized fall hazards associated with the proper use and placement of ladders.  This most recently occurred on April 17, 2017 on the jobsite located at 5323 N 134th Ave, Omaha, NE 68164.  The employer had untrained employees using an extension ladder as a means to access and egress the commercial roof which has an eave to ground height of approximately 9 feet. The two ladders being used were placed on an uneven and not secured to the building in order to prevent a fall.
