29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:    a.  On or about April 24, 2017, at the east stairs between the 1st and 2nd floors, a handrail was not present at a flight of stairs.   Employees were exposed to tripping hazards when using the stairs.

29 CFR 1926.1052(c)(4)(iii): Intermediate vertical members, such as balusters, used in stairrail systems between posts were more than 19 inches (48 cm) apart:    a.  On or about April 24, 2017, at the east stairs between the 1st and 2nd floors, balusters for a flight of stairs were greater than 19 inches apart.  Employees were exposed to tripping hazards when using the stairs.

29 CFR 1926.501(c)(1): When employees were exposed to falling objects, the employer did not have each employee wear a hard hat and did not erect toeboards, screens, canopy structures or barricades to prevent objects from falling from higher levels:    On or about April 24, 2017, at the east stairs, stair landing between the 1st and 2nd floors, a toeboard was not present at the interior wall opening.   Employees at the first floor were exposed to struck-by hazards from objects falling through the unprotected edge.
