On or about April 24, 2017, at the east stairs between the 1st and 2nd floors, a handrail was not present at a flight of stairs. Employees were exposed to tripping hazards when using the stairs.

On or about April 24, 2017, at the east stairs between the 1st and 2nd floors, balusters for a flight of stairs were greater than 19 inches apart. Employees were exposed to tripping hazards when using the stairs.

29 CFR 1926.501(c)(1): When employees were exposed to falling objects, the employer did not have each employee wear a hard hat and did not erect toeboards, screens, canopy structures or barricades to prevent objects from falling from higher levels: On or about April 24, 2017, at the east stairs, stair landing between the 1st and 2nd floors, a toeboard was not present at the interior wall opening. Employees at the first floor were exposed to struck-by hazards from objects falling through the unprotected edge.
