OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards while working on the Roto mold platform:    a) Roto mold platform:  A CM Lodestar 2 ton overhead hoist (underhung) (Model RR/SN 0607XL) was operated while employees were working beneath the load, on or about April 26, 2017.    b) Roto mold platform:  The frame of the mold being lifted by a CM Lodestar 2 ton overhead hoist (underhung) (Model RR/SN 0607XL) was not clear of the chain canister causing the mold to be obstructed and unable to move freely during the lifting process, on or about April 26, 2017.     c) Roto mold platform: A CM Lodestar 2 ton overhead hoist (underhung) (Model RR/SN 0607XL) was operated with a hook without a safety latch, on or about April 26, 2017.     Abatement Certification and Documentation Required.
