Employees were exposed to struck-by hazards while the excavator was in operation.

Get'er Dug, LLC worksite located at Blackhawk Dr. Lot 13, Waterville, Ohio: On or about April 26, 2017, the employer did not ensure an employee was adequately protected from cave-ins while installing a sanitary sewer line in an unprotected trench approximately 8.00 feet deep. The trench consisted of 'B' soil with near vertical side walls.
