29 CFR 1910.269(l)(8)(v): The employer did not ensure that each employee exposed to hazards from electric arcs wore protective clothing and other protective equipment that had an arc rating greater than or equal to the heat energy estimated under paragraph (l)(8)(ii) of this section whenever that estimate exceeded 2.0 Cal/cm2. Worksite: The employer did not ensure that the employees required to work on or near energized electrical equipment were wearing appropriate types of personal protective equipment (PPE) at all times to protect from the hazard of electrical arc flash. On or about 04/26/2017, one of employees sustained severe facial burns as the result of an arc flash/blast while performing tasks, such as (but not limited to) testing and replacing energized electric utility meters.

29 CFR 1910.269(a)(2)(i)(A): Each employee was not trained in, and familiar with, the safety-related work practices, safety procedures, personal protective equipment, and other safety requirements in this section that pertain to his or her job assignments. Worksite: The employer did not ensure that the employees who were required to work on or near energized electrical equipment/parts were trained in and familiar with the requirements for personal protective equipment related to the job tasks/assignments. On or about 04/26/2017, one of employees performing tasks, such as (but not limited to) testing and replacing energized electric utility meters without wearing appropriate types of personal protective equipment (PPE) sustained severe facial burns as the result of an arc flash/blast.
