29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Saw 1: On or about April 24, 2017, and at times prior, employees were exposed to struck-by hazards while utilizing an unguarded cutoff saw used for cutting mild steel and aluminum within the production.      b) Saw 2: On or about April 24, 2017, and at times prior, employees were exposed to struck-by hazards while utilizing an unguarded cutoff saw used for cutting mild steel and aluminum within the production.      c) Press 1: On or about April 24, 2017, and at times prior, employees were exposed to caught in hazards while utilizing an unguarded drill press used on mild steel and aluminum within the production.      d) Press 2: On or about April 24, 2017, and at times prior, employees were exposed to caught in hazards while utilizing an unguarded drill press used on mild steel and aluminum within the production.      e) Press 3: On or about April 24, 2017, and at times prior, employees were exposed to caught in hazards while utilizing an unguarded drill press used on mild steel and aluminum within the production.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a) All Pro Off Road, Inc.: On or about April 24, 2017, and at times prior, employees were exposed to unknown and unlabeled containers of brownish black liquids. The employer failed to ensure that each container of hazardous chemicals in the workplace was labeled with the product identifier and words, pictures, symbols, or combination thereof, which provides at least general information regarding the hazards of the chemicals.  b) All Pro Off Road, Inc.: On or about April 24, 2017, and at times prior, employees were exposed to unknown and unlabeled containers of brownish black liquids. The employer failed to provide the required safety data sheet for each hazardous chemical in use.  c) All Pro Off Road, Inc.: On or about April 24, 2017, and at times prior, employees were exposed to unknown and unlabeled containers of brownish black liquids. The employer failed to provide effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.
