29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  (a) Green Gable Roofing LLC, at 1514 Main Street, Louisville, CO: On 4/26/17 the employer did not ensure that eye protective equipment was used when operations presented potential eye injury.  Workers were re-roofing a single story home.  Workers used tools to remove asphalt shingles and tar paper.  Workers used pneumatic nail guns to install tar paper and shingles.  The employer did not ensure that workers wore eye protection.  This condition exposed workers to a flying object hazard.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure:  (a) Green Gable Roofing LLC, at 1514 Main Street, Louisville, CO: On 4/26/17, the employer did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure.  Workers were re-roofing a single story home.  The workers were not protected from a fall.  This condition exposed the workers to an approximate 8.5 foot fall hazard.  (b) Green Gable Roofing LLC, at 1518 Main Street, Louisville, CO: On 4/26/17, the employer did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure.  A worker was re-roofing a two story building.  The worker was not protected from a fall.  This condition exposed the worker to an approximate 15 foot fall hazard.

29 CFR 1926.503(a)(1): The employer did not provide a training program enabling recognition of and appropriate protective procedures for fall hazards for each employee who might be exposed to fall hazards:  (a) Green Gable Roofing LLC, at 1514 Main Street and 1518 Main Street, Louisville, CO: On and preceding 4/26/17, the employer did not provide a training program enabling recognition of and appropriate protective procedures for fall hazards for each employee who might be exposed to fall hazards.  Workers were re-roofing a single story home and a two story business.  The employer did not provide fall protection training to the workers.  This condition exposed the workers to approximate 8.5 foot and 15 foot fall hazards.
