29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about April 25, 2017, the employer failed to ensure employees cutting wood product and transferring it to the roof via a ladder lift were protected against the product falling and striking them in the head.

29 CFR 1926.102(a)(1):  The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  On or about April 25, 2017, the employer failed to ensure employers operating a Makita Circular Saw were protected against being struck in the face and eyes by flying debris while cutting wood.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a.  On or about April 25, 2017, the employer failed to ensure employees engaged in residential construction to include sheeting were protected against falling by utilizing an effective fall protection devices/systems. Employees were exposed to falls of approximately 20 feet while performing sheeting operations.  b.  On or about April 25, 2017, the employer failed to ensure employees engaged in residential construction to include framing were protected against falling by utilizing an effective fall protection devices/systems. Employees were exposed to falls of approximately 20 feet while performing framing operations.  c.  On or about April 25, 2017, the employer failed to ensure employees exposed to approximately a 15 foot fall through window openings while engaged in residential construction were protected through the use of a guardrail system.
