29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:      a) On or about 04/26/17, on roof of the Home Depot Store,  4405 Milestrip Rd., Blasdell, NY:  A thick yellow extension cord was plugged into the circular 120/240 volt receptacle on a Champion 9000 peak watt portable generator. The female end of that cord had four 120 volt receptacles. Other extension cords were plugged into those receptacles.  One of those extension cords supplied power to a DeWalt, Model  DW272,  VSR Drywall Screwdriver which an employee used to fasten screws. There was no Ground Fault Circuit Interrupter on that circuit nor did the employer have an assured equipment grounding conductor program.        NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.502(f)(1): Warning lines were not erected around all sides of the roof work area:        a) On or about 04/26/17, on roof of the Home Depot Store,  4405 Milestrip Rd., Blasdell, NY:  Employees  were installing a TPO Rubber Roofing System  on the roof of the store. The roof was flat and the employer chose to use a combination of warning line system and safety monitoring system in accordance with OSHA Construction Standard 1926.501(b)(10).  In the northwest corner of the roof, the warning line had been taken down and employees were exposed to falls of over 20 ft.  In other areas of the roof, the warning system was in place, but there were deficiencies  in the set-up of the warning line, such as not being flagged every 6 feet in some areas, being  less than 6 ft. from the edge in other areas, and the height of the warning line was greater than  39 in. above the roof  in places.      NO ABATEMENT CERTIFICATION REQUIRED
