29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a)  At 11412 Gulf Freeway, Houston Texas, where employees working on a steep roof, approximately 29 feet above the ground, were not protected from falling by any type of a fall protection system.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder.       a)  At 11412 Gulf Freeway, Houston TX, an extension ladder used to access the roof did not extend at least 3 feet above the upper landing surface and was not secured.
