(a) Delta Building Center LLC dba Overholt Truss Co LLC, 1796 Hwy 50, Delta, CO 81416: The employer did not report a work-related in-patient hospitalization and amputation incident to OSHA within 24 hours. On April 10, 2017 an employee sustained an amputation injury while operating a component saw. The employer reported the incident to OSHA on April 21, 2017.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven feet or less above floors or platforms were not enclosed: (a) Delta Building Center LLC dba Overholt Truss Co LLC, 1796 Hwy 50, Delta, CO 81416: On or before April 10, 2017 the employer did not protect employees from injury, in that the drive sprocket wheels and chains on the component saw were not guarded. This condition exposed operators and other employees to ingoing nip point hazards.
