29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  On or about April 27, 2017, at the above addressed site, employees were exposed to a fall hazard of approximately 10 feet when performing paint preparation work from the basket of an aerial lift while not wearing harnesses with lanyards attached to the boom or basket of the lift.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net  systems, or personal fall arrest systems:  On or about April 27, 2017, at the above addressed site, an employee was exposed to a fall hazard of approximately 10 feet when performing paint preparation work from a walking/working surface while not being protected from falling to the lower level.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work  shift to employees when they were in their work area(s):  On or about April 27, 2017, at the above addressed site, employees were using chemicals including but not limited to Benjamin Moore Ultra Spec Exterior Satin Paint, Benjamin Moore Ultra Spec Exterior Gloss Paint and Lighthouse Products Siliconized Latex Adhesive Sealant while the employer did not maintain the safety data sheets for those products at the worksite.
