29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee: (a)A&A Construction, LLC @ 1335 Elati Street, Denver, CO 80204: On or before April 17, 2017, as part of injury/accident prevention responsibilities under its safety program, the exposing employer did not ensure each employee performing sheathing activities on the 1st floor of a multi-family residential apartment complex was using individual anchorages attachment of personal fall arrest equipment as required under the standard. Two employees were observed being tied-off to the same anchorage point used for attachment of Personal Fall Arrest Systems (PFAS). This condition exposed employees to the hazard of falling approximately 12 feet to ground.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed: (a) A&A Construction, LLC @ 1335 Elati Street, Denver, CO 80204: On or before April 17, 2017, as part of injury/accident prevention responsibilities under its safety program, the exposing employer did not ensure that the extension ladder used by the employees to gain access to the first floor was not extended at least three feet above the floor deck to provide safe access. This condition exposed employees to the hazard of falling approximately 12 feet to ground.
