(b) 400 Seaks Run Road, Glen Rock Pennsylvania, 50 gallon reactor - The employer did not ensure that when employees preformed maintenance on a machine that a device was affixed to each energy source, on or about April 27, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.147(f)(1): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions described in 1910.147(f)(1(i) through 1910.147(f)(1(v) were not followed: (a) 400 Seaks Run Road, Glen Rock Pennsylvania, 50 gallon reactor - An employee repositioned the machine to clean grease from the gear. The employer did not develop a procedure for the repositioning or testing of machines, equipment or components to be used in conjunction with the machine specific procedure, on or about April 25, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
