29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:  a)  At the job site located at 1445 Satellite Blvd, Suwanee GA, on or about April 27, 2017, an employee was observed working/walking from a fabricated frame scaffold with metal decking putting bricks on a wall.  The scaffold was not fully decked, exposing the employee to fall hazard of about 12 ft. to the ground.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along open sides and ends of platforms:  a)  At the job site located at 1445 Satellite Blvd, Suwanee GA, on or about April 27, 2017, an employee was observed working/walking from a fabricated frame scaffold with metal decking putting bricks on a wall. It had no top and mid rails, exposing the employee to fall hazard of about 12 ft. to the ground.

29 CFR 1926.454(a): The employer did not have each employee who performed work on a scaffold is trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used, and to understand the procedures to control or minimize those hazards.   a)  At the job site located at 1445 Satellite Blvd, Suwanee GA, on or about April 27, 2017, an employee working from the second level platform of a scaffold was not trained to recognize fall hazards associated with scaffolds, exposing employees to a 12 feet fall hazard.
