29 CFR 1910.157(d)(4): Portable fire extinguishers for use on Class B fires were not distributed so that the travel distance from the Class B hazard area to any extinguisher was 50 feet or less: Employees working in or near the flammable storage were exposed to fire hazards in that the nearest fire extinguisher was greater than 50 feet away from the flammable liquids storage and not readily accessible in the event of an emergency.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: Employees were exposed to fire, and explosion hazards in that: a) Oxygen cylinder were not secured to prevent falling or being knocked over in the Fire Station Bay. b) The name of the gas to be stored was not prominently posted in the cylinder storage area of the Fire Station Bay. c) Several small compressed gas cylinders were stored with flammable liquids in the flammable storage. The Department of Interior, National Park Service, Grand Canyon Facility Management Division was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.101(b), which was contained in OSHA inspection number 994371, citation number 01, item number 003 and was affirmed as a final order on February 4, 2015, with respect to a workplace located at building 1575, Shuttle Bus Road, Grand Canyon, Arizona 86023.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: Employees working in the Fire Station Bay were exposed to fire hazards in that the fire extinguishers were not mounted and identified to be readily accessible in the event of an emergency. The US Department on Interior, National Park Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(c)(1), which was contained in OSHA inspection number 1086115, citation number 1, item number 1 and was affirmed as a final order on 2/25/2016, with respect to a workplace located at 1 Black Canyon Road Essex, CA 92332.
