29 CFR 1910.28(b)(1)(i): (1) Unprotected sides and edges. (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: 29 CFR 1910.28(b)(1)(i)(A): Guardrail systems; or in the alternative, 29 CFR 1910.28(b)(1)(i)(B): Safety net systems; or in the alternative, 29 CFR 1910.28(b)(1)(i)(C): Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems. a. On or before April 27, 2017 where employees worked from elevated platforms, the chains on the access areas on each side or the platforms were not installed, thereby exposing the employees to falls from heights of 8 feet. b. On or before April 27, 2017 where employees were preparing to work from elevated platforms, the employees walked out unprotected onto the unguarded platform to raise the guardrail systems, thereby being exposed to falls from heights of 8 feet. c. On or before April 27, 2017 where employees worked from elevated platforms, the pins that were required to hold the guardrail stanchions in the upright position were not used, thereby exposing the employees to falls from heights of 8 feet. d. On or before April 27, 2017 where employees worked from elevated platforms, the inside edge of the elevated platform, between the manufactured home and the end of the platform was unguarded, thereby exposing the employees to falls from heights of 8 feet.

The top edge height may exceed 45 inches (114 cm), provided the guardrail system meets all other criteria of paragraph (b) of this section (see Figure D-11). a. On or before April 27, 2017 where employees worked from elevated platforms, the guardrail system top chain between the stanchions sagged to 26 inches, thereby exposing the employees to 8 foot fall hazards.
