On or about April 27, 2017, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems: a. On or about April 27, 2017, at the above addressed jobsite, employees performing roofing operations on the roof, eighteen (18) feet and nine (9) feet above ground, were not protected from falls, thereby exposing the employees to fall hazards. P+B Rebuilders LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1162122, citation number 1 item number 1 and was affirmed as a final order on August 3, 2016, with respect to a workplace located at 130 Parkside Drive, Palos Park, IL 60464. P&B Rebuilders, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 911653, citation number 1, item number 1 and was affirmed as a final order on August 22, 2013, with respect to a workplace located at 308 North Water, Thornton, IL 60476.
