29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used for access. Site: Front of building at 107A Halsey Street, Brooklyn, NY 11216. On or about 4/28/2017; a) Employees worked on a 6 frames high, tubular welded frame scaffold and it did not have a safe means of access. An employee climbed the cross braces and scaffold frames to access the scaffold.

Site: Front of building at 107A Halsey Street, Brooklyn, NY 11216. On or about 4/28/2017; a) Employees working on the 4th level of a tubular welded frame scaffold. No guardrails were installed along the two open ends and the open back side.

29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems: Site: Front of building at 107A Halsey Street, Brooklyn, NY 11216. On or about 4/28/2017; a) An employee hoisting a bucket of cement from the ground floor to the platform on the 4th tier of the scaffold was exposed to a fall hazard of approximately 24 feet. The employee was not provided with fall protection.
