29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. Employees in the production area were exposed to amputation hazards. There are no machine specific lockout tagout procedures for machines, such as, but not limited to, line 2 conveyor belt roller assembly, known to company as the AMS filling machine.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: Employees were exposed to serious amputation, crushing and struck-by injuries, in that employees had not been trained to understand and demonstrate the knowledge and skills required to safely lock and tag out the energy sources for the various types of machinery and equipment. Equipment such as, the AMS line 2 filling machine, serial number A-400266E.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees were exposed to amputation and crushed-by hazards. Employees working in the production area were exposed to unguarded rotating parts of equipment. Machine guarding was not provided for the line 2 conveyor belt roller assembly, known to company as the AMS filling machine, located less than 7 feet from the walking working surface.
