29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    229 Plantation St, Worcester, MA    On April 28, 2017 and at times prior, the employer did not ensure that employees wore eye protection when they used Hitachi nail guns.  The nail guns had warning labels that read, "Operators and others in work area MUST wear eye protection (safety glasses with side shields.)"

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     229 Plantation St, Worcester, MA    On April 28, 2017, the employer and other employees were exposed to fall hazards of more than eleven feet while they were performing residential roofing work.  The employer and at least one other employee were on the roof wearing personal fall arrest harnesses but neither was connected to the installed lanyards.

The employer and at least one other employee were on the roof, but their personal fall arrest system lanyards were not properly adjusted to prevent a fall more than 6 feet.
