The employer is failing to protect employees from fall hazards when conducting roofing operations on a commercial structure.  This was most recently documented at 5532 Center St, Omaha NE on or about April 4th, 2017 when employees were observed installing roofing membrane material on a low slope roof while working greater than 6 feet above the ground without fall protection in use.
