29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not  independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee:    a) North side of roof, on or about 4-26-17:  Employees were working on a flat roof with unprotected sides and edges, and were connected to a sheet metal hood that was not designed and rated to withstand a 5,000 pound shock load, exposing the employees to a fall hazard of 21 feet 10 inches.     b) North side of roof, on or about 4-26-17:  Employees were working on a flat roof with unprotected sides and edges, and were connected to a nail in the roof that was not designed and rated to withstand a 5,000 pound shock load, exposing the employees to a fall hazard of 21 feet 10 inches.
