On or about May 1, 2017, in production area, an employee was required to perform maintenance on the slitter machines. The company's written lockout program did not include specific energy control procedures for isolating the energy sources for the slitter machines. Employees were thereby exposed to the hazards associated with unexpected energization of equipment
