29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from the hazards such as those created by point of operation, in-running nip points, rotating parts, flying chips and sparks:     The employer is failing to protect employees from the hazards associated with broken bones and potential amputations.  Employees are required to operate the vertical band saw with the portion of the blade below the cutting surface open and not guarded creating a laceration/amputation hazard along with exposure to the in-running nip point on the pulley of the saw.  One instance, which was identified on April 27, 2017, involved the vertical band saw in the maintenance area in the facility located at 4949 S.30th St., Omaha, Nebraska.

29 CFR 1910.147(c)(7)(i)(C):  Other employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:   The employer is failing to ensure that all employees in the facility are adequately trained in regards to this standard.   Employees that are required to potentially exposed to electrical hazards that are not normally present in their work environment have not been trained as to the procedures and the and the prohibition relating to the attempts restart or re-energize equipment which are locked out or tagged out.  One instance, which was identified on April 27, 2017, involved the office staff not receiving this training in the maintenance area in the facility located at 4949 S.30th St., Omaha, Nebraska.

29 CFR 1910.1200(e)(1)(ii): The written hazard communication program did not include the methods the employer will use to inform employees of the hazards of non routine tasks, and the hazards associated with chemicals contained in unlabeled pipes in their work areas: (Construction Reference:   The employer is failing to ensure that the company has a written hazard communications (HAZCOM) program which contains all of the elements required by this subpart.  There is no provision in their written program as to how to deal with non routine or unlabeled piping in their facility.  One instance, which was identified on April 27, 2017, involved this provision in the facility located at 4949 S.30th St., Omaha, Nebraska.
