29 CFR 1910.147(c)(4): Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) Case Pick Structure - On or about 4/15/2017, the employer does not have specific procedures for locking out the Multilevel Vehicle Conveyor (MVC) during an employee confined space entry for removal of debris from an untenanted charging pad which exposed the employee to amputation and caught in hazard from the MVC which moves vertically and intersects with each level of the structure during the entry.         - Up to 8 bots move on each level with a speed capacity of 26mph exposing employees to struck by hazards.         - The Multilevel Vehicle Conveyor (MVC) moves vertically and intersects with each level of the structure, which presents an amputation and caught in hazard to employees maintaining the charging pads.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:      a) Symbotic structure - On or about April 15, 2017, the employer did not  have lockout/ tagout devices affixed to the MVC (multilevel vehicle conveyor). An employee sustained serious hand injuries when his hand was caught in between the structure and the MVC while working on a bot charging station.

29 CFR 1910.146(k)(1)(ii): The employer did not evaluate a prospective rescue service's ability, in terms of proficiency with rescuerelated tasks and equipment, to function appropriately while rescuing entrants from the particular permit space or types of permit spaces identified.    a) Warehouse, 1500 Corporate Blvd, Newburgh, NY - On or about 4/15/2017, an employee performing troubleshooting and maintenance activities on a bot charging station in a permit required confined space, sustained serious hand injuries when his hand got caught in between the structure and the MVC (multi-level vehicle conveyor). The rescue services identified on the permit was a 100% volunteer department and did not have a Confined Space rescue team.

The rescue services listed on the permit was a volunteer department, which could not be reached throughout the inspection.
