29 CFR 1926.453(b)(2)(vi): Boom and basket load limits specified by the manufacturer were exceeded: (a) On or about April 26, 2017, two employees working in a Genie S-65 aerial lift allowed another man into the aerial lift and exceeded the maximum occupant load limits of the equipment and was extended approximately 30 feet above ground. 29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: (a) On or about April 26, 2017, and at times prior thereto, employees observed working at elevated heights in an aerial lift demonstrated the lack of understanding of the hazards associated and/or the awareness of how to control and/or minimize the risk of the potential hazards associated with overloading the equipment.
