29 CFR 1926.405(g)(2)(iii): Flexible cords were not used in continuous lengths without splice or tap:    JOB SITE: 8000 Ivory Avenue, Saint Louis, MO 63111, on or about 5/2/2017, a yellow electrical flexible cord was being used to power the concrete mixer and the cord was not used in continuous lengths without splice or tap.  The flexible cord had been spliced in multiple locations on the cord.  The workers were exposed to an electrical hazard.
