Warehouse and throughout the facility- On or about May 02, 2017, The following forklift trucks were not kept in a safe working condition, thereby exposing employees to the hazard(s) associated with a moving forklift truck. 1. Toyota #2, Serial Number 95347, Model Number 7FGCU25- Left back-up light inoperable. 2. Toyota #24, Serial Number 20064, Model Number 8FGCU25- Left back-up light inoperable, right front tire excessively worn and gouged, emergency brakes inoperable, strobe light and vehicle back-up alarm inoperable. 3. Toyota #1, Right rear back-up light inoperable, left front headlight inoperable, right rear tires excessively worn and emergency brake inoperable.
