Sufficient access and working space was not provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment: Instance a. Forklift battery charger - On or about April 26, 2017, the employer failed to maintain the Power Flow (Model 18P225B22 S/N F31621) forklift battery charger to ensure that materials were not allowed to be stored on the charger or directly in front of it. Instance b. Forklift charger disconnect - On or about April 26, 2017, the emergency disconnect for the Power Flow (Model 18P225B22 S/N F31621) battery charger was not maintained to ensure that it was free of all storage and debris to allow easy access at all times during emergencies or maintenance procedures.
