29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following; a guardrail system, safety net system or a personal fall protection system:   a) On or about May  3, 2017, at the above located workplace, employees  were required to add cellulose and clay to the product while working on a work platform  on the second level. The work platform had an open side without a guardrail (or the equivalent). The employees were thereby exposed to a fall hazard of approximately 10-12 feet.

29 CFR 1910.28(b)(6)(i): Each employee less than 4 feet (1.2 m) above dangerous equipment was not protected from falling by a guardrail system or a travel restraint system when the equipment was not covered or guarded to eliminated the hazard:    a) On or about May  3, 2017, at the above located workplace, employees were required to work around a 3.0 feet by X 3 feet floor opening. The floor opening was over an uncovered horizontal ribbon mixer. The mixer's ribbons were less than 1 foot below floor level. The employees were thereby exposed to fall hazards and/or crushing hazards.

29 CFR 1910.29(b)(1): The employer did not ensure that each guard rail system had a top edge height of top rails, or equivalent guardrail system members, that was 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface:     a) On or about May 3, 2017, at the above located workplace, employees on the second level just west of the horizontal ribbon mixer were required to work on an open-sided floor which had a top rail that was 30 inches above the floor. Employees were thereby exposed to a fall hazard of approximately 10 feet.

29 CFR 1910.29(b)(2): The employer did not ensure that each guard rail system had a  midrail, screen, mesh, intermediate vertical member, solid panel, or equivalent intermediate member installed between the walking-working surface and the top edge of the guardrail system:   a) On or about May 3, 2017, at the above located workplace, employees on the second level were required to work at and around a stairway's floor opening. The floor opening guardrail did not have a  midrail, screen, mesh, intermediate vertical member, solid panel, or equivalent intermediate member installed between the walking-working surface and the top edge of the guardrail system. The employees were thereby exposed to a fall hazard of approximately 10 feet.     b)On or about May 3, 2017, at the above located workplace, employees on the second level were required to work just south west of the stairway. The guardrail on the south side of the platform did not have a midrail, screen, mesh, intermediate vertical member, solid panel, or equivalent intermediate member installed between the walking-working surface and the top edge of the guardrail system. The employees were thereby exposed to a fall hazard of approximately 10 feet.

29 CFR 1910.29(b)(10): In a hoist area the employer used chains that did not provide a level of safety equivalent to guardrails:    a) On or about May 3, 2017, at the above located workplace, employees were required to work in the hoist area just north of where cellulose and clay were added to the product. The hoist area floor edge was guarded with a single chain that did not provide employee safety equivalent to guardrails. The employees were thereby exposed to a fall hazard of approximately 10 feet.
