29 CFR 1926.703(d)(1): Reinforcing steel for walls, piers, columns, and similar vertical structures was not adequately supported to prevent overturning and to prevent collapse: (a) Winfield, PA: A thirty-eight (38) foot high rebar tower set on top of a bridge pier was not adequately supported to prevent collapse when the guy wires securing it in place were removed, causing the rebar tower to collapse, exposing employees to a potential injury, on or about May 1, 2016. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
