29 CFR 1926.503(a)(2)(i): 29 CFR 1926.501(a)(2)(I):  The employer shall ensure that each employee has been trained , as necessary, by a competent person, in the nature of fall hazards in the work area:   On the west side of the construction site, the employer did not ensure that the employees working from a height of approximately 9 feet without fall protection was trained to recognize and correct hazards associated from falls.

29 CFR 1926.102(a)(1): 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   On the west side of the construction site, employees engaged in roofing activities operated pneumatic nail guns without wearing eye and face protection were exposed to struck-by hazards and eye injuries.   Jose Santos Barrera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1100710, citation number 001, item number 1 and was affirmed as a final order on 3/7/2016, with respect to a workplace located at 907 Enchanted Wood, Arlington, Texas 76005.

On the west side of the construction site, employees engaged in roofing activities without fall protection were exposed to a fall hazard of approximately 9 feet to the ground below.   Jose Santos Barrera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1182929, citation number 001, item number 1 and was affirmed as a final order on 11/25/2016, with respect to a workplace located at 349 Bessie Coleman., Waxahachie, Texas 75165.  Jose Santos Barrera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1129210, citation number 001, item number 1 and was affirmed as a final order on 4/18/2016, with respect to a workplace located at 4105 Rose Spirit ST., Arlington, Texas 76005. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    On the west side of the construction site, employees used 2 ladders that did not extend 3 feet above the landing to access and egress the roof and were exposed to a fall hazards to the ground below.
