NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    a- 12 America Avenue, Lakewood NJ - A company employee was exposed to fall hazards, in excess of 25 feet to the ground surface below, while performing red masonry brick installation from a prefabricated, tubular, welded framed scaffold that was missing guardrails and end stops on the scaffold's walking/working surface.  On the day of the worksite inspection, the exposed employee and the company owner had not received scaffold hazard recognition and minimization training by a qualified person.  Condition noted on or about 05/03/17.
