29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways: a) This violation occurred on or about April 26, 2017, on the second floor, at the opening of an elevator shaft, when the employer did not ensure that an employee working from an A-frame ladder to trim fiberglass insulation board on the inside of an elevator shaft had been trained by a competent person in the proper use of the ladder; exposing the employee to a fall hazard of approximately fifteen (15) feet six (6) inches to the ground below.
