The dispensing valve for the flammable liquid was not self-closing.

The approximately 15 gallon drum was not grounded to provide the dissipation of static electricity associated with dispensing the liquid from the drum to smaller containers.

The metal drum was not bonded to the smaller containers, during transfer, to provide dissipation of static electricity while dispensing the flammable liquid.
