29 CFR 1910.1450(e)(3)(i): The employer's Chemical Hygiene Plan did not include the standard operating procedures relevant to safety and health considerations to be followed when laboratory work involved the use of hazardous chemicals:    a) 23 Frontage Road, Andover MA 01810.     On or about May 1, 2017, the employer did not ensure that their chemical hygiene plan included standard operating procedures for the disposal of incompatible wastes. Nitric and hydrochloric acids were mixed together in a vessel that previously contained organic materials, ultimately resulting in the explosion of the vessel in which the acids were kept.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1904.19(d).
